Here's another Final Fantasy one. FFXIII, there's an achievement called "Treasure Hunter" you have to have owned every weapon and item in the game. It's an incredibly time consuming achievement which requires a lot of grinding thanks to the retarded crafting system in FFXIII.
I had a check list, every time I crafted a weapon or item I meticulously checked the mother fucker off.
Fuck knows how many hours of Platinum Ingot farm runs on Adamantoises later I thought I was down to my last weapon.
Nothing happened.
No Trophy. It was the most deflating thing I ever experienced in a singleplayer game. FFXIII will forever sit at 91% completion, because even though I have an old save I could make another attempt at it on, my research into this Trophy tells me that this one is glitchy enough for it to be really not worth the hours it would take.
I had the multiplayer horror of booting up a game of BC2 back in the day and finding I only had rank 1 gear, but it was an EA-online server side issue that just happened semi-regularly. I even got good with the bag of shit AEK-971 Assault rifle you have to start with in BC2 (I find it amusing that this gun was one of the 1st "OP" guns in BF3 too).

I sent one of the Treyarch guys a PM on the BO2 forums, and he (mostly) got all my stats back.
I had played 75 hours, but I'm basically back where I was when I had played 69 hours, so at least I don't have to start again from scratch.
Anyway,
Duty calls...

The worse thing I ever had was due to Half-Life 1 and Sierra On-Line (which was a heinous pile of software you were forced to install to play the game back when it was first out).
Basically, when uninstalling it what it did was remove everything in the folder it was installed in (in my case C:\Games) that was put there after it. Also, bear in mind this was early 2000's, before games started saving data to your user folder.
What did I lose? Well, 75 hours of PST (basically infront of the last dungeon) 55 hours of BG2 (Had just left the underdark) 90 hours of Morrowind (I had beat the game, beat the first addon Tribunal and took down vivec, had 100 in all stats legitimately as you didn't have a cap on training) and probably a bunch of other installs. But mostly, it's that Morrowind save I miss the most.
Never properly played any of the games again outside of PST, and even that I have only put like 10 hours in over the years.

PUG`s in online games 97% of the time ><
Examples:
Gears Of War 3 Horde mode, people disconnecting on wave 9,19,29,39, etc...
WOW, players refusing to tank, heal, DPS, pulling the boss before grp is ready,or whatever BS because of some imagined slight of their nerd honour or something like that.

I had my old 60GB PS3 die in when I was close to the end of both Demon's Souls and Persona 4. Losing Persona 4, especially was bad, because I had never thought of just backing up my data on a USB stick until then. Now, even though I haven't had any problems with my slim for the few years I've had it, I still back up any saves that aren't locked after every session.
As far as a few other things in this thread...
I hated Eternal Sonata with a passion. The gameplay was kinda fun, but everything else was shit. The ending wasn't even as bad as some of the cutscenes in the middle, like when one character who is supposed to be dying pulls a Bugs Bunny and gives a thirteen minute monologue during her supposed-final moments, and then keeps getting back up to say more.
Although at least it had an ending, very few people have ever beaten Battletoads, so very few know that the ending is one step away from, "Ha! You dumb fuck, look how much of your pre-teen years you've wasted to get to this! A winner is you!"
If there ever was a company I can say I was a fanboy of, it would definitely have been Sega. Part of me still wishes that Sega would announce they were getting back into the hardware race, because the franchises they own are so much better than the franchises any other first party company own. Yakuza, Valkyria Chronicles, Shining Force, Bayonetta...all on one system? Fuck yeah, I'd buy that. Of course, with Sega's marketing department being a guy with a bag over his head and his fly open, they'd sell about three units and probably file for bankruptcy afterwards.

My PC hard drive was getting full so I wanted to uninstall a game. I uninstalled 1 game...later realizing it had somehow completely wiped my games folder :( Needless to say, It deleted over 100GBs of games and the game saves! I know I should back up my hard drives, just hadn't found a good deal on another 1TB drive...Jeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeeez

@Hunam
Oh I had that too, forgotten which game it was but Sierra does ring a bell. Tell it to install in my C:\Games directory and the installer clears it first.Posted 5 months ago # -
I remember one time when I really cried, Final Fantasy VIII, I was in the third disc, I was going to open up some space for another game (doesn't even remember what it was) and acidentally deleted the FF save game, well, the window of my room still have bent metal from my punches (here it's normal to have metal windows).
And there was the last boss of Pitfall 3D, OH MY GOD, I don't even remember what it was, but all the pain is still inside me.
